Please be sure to email a PDF of your Childs most recent vaccination records.

If these forms are not completed and we have not received a copy of your child immunization records your child is legally not permitted to attend The Willow School. Completing your Childs paper work before their first day ensures that we maintain our legal responsibilities to the State of Maine and to one another!
